  • Hypoglycemia Risk Screening Tool

    Algorithm that when applied to EHR data, identifies a cohort of patients with T2D aged 65 and older who are at risk of medication-related hypoglycemia. The Screening Tool utilizes an evidence-based algorithm to enable providers to identify who within their patient panel is at risk for hypoglycemia.

  • Hypoglycemia Reduction Clinical Decision Support Tool

    An evidence-based, point-of-care decision support tool to guide providers in patient assessment, care planning, and monitoring and ongoing evaluation of patients with a focus on reducing risk of hypoglycemia through the individualization of A1c goals and/or changes in medication management.
  • Hypoglycemic Confidence Scale
    Behavioral Diabetes Institute | Study Endpoint Instrument
    A validated, 9-item self-report scale that examines the degree to which people with T2D feel able, secure, and comfortable regarding their ability to stay safe from hypoglycemic-related problems.
  • TRIM-HYPO
    Novo Nordisk | Study Endpoint Instrument 
    A validated patient survey that assess the impacts of non-severe hypoglycemia on an individual. The survey is divided into 5 domains: daily function, emotional well-being, diabetes management, sleep disruption, and work productivity.
