Patient Education
These resources are intended to help educate patients about their risk for hypoglycemia and ways to treat episodes. They are organized by topic and scored for literacy level. The SMOG Index is used - which computes readability based on the number of syllables per word. The number indicates the reading level of each resource.         Alzheimer’s Association
        Toolkit that outlines a recommended process for efficiently identifying patients with probable cognitive impairment during the Medicare Annual Wellness visit and options for different validated  tools that a primary care provider can use to conduct their assessment of a patient's cognition. 
        - Montreal Cognitive Assessment (MoCA)
        Designed as a rapid screening instrument for mild cognitive dysfunction. It assesses different cognitive domains: attention and concentration, executive functions, memory, language, visuoconstructional skills, conceptual thinking, calculations, and orientation. It takes approximately 10 minutes to administer.
